team membership index page needs breadcrumbs

Bug Description
The team membership index page, i.e. the membership page for a person in a team, e.g. from xx-add-member.txt:

has no breadcrumbs. This is because there is no +hierarchy adapter for the view, since it is subordinate to the team. I've spent some time trying to add one, but for the sake of expediency in converting our 3.0 pages, I'm punting on adding breadcrumbs for this page.
Because the page has no breadcrumbs, it has no reversed breadcrumb page title. The new page title is taken nearly verbatim from the old pagetitles.py entry for the page.
