PPA "Activate" button is available even if I haven't accepted the terms of service

Bug Description
1. As someone who doesn't have a PPA, go to <http://
2. Without checking the "I have read and accepted the PPA Terms of Service" checkbox, click "Activate".
What happens:
* "There is 1 error."
* "PPA Terms of Service must be accepted to activate your PPA."
What should happen:
2. If JavaScript is available, the "Activate" button should be disabled whenever the checkbox is unchecked.
