Inconsistencies in terms and processes for {approv,accept}ing/declining various types of nomination

Nominations of at least three different types appear in Launchpad, with different terminologies and renderings:

 - Bug nominations on /distro/distroseries/+nominations: Radio buttons (Accept/Decline/No change) in a single column with titles up the top.
 - Team member proposals on /~team/+editproposedmembers: Radio buttons (Approve/Decline/Hold) with labels next to each.
 - Unapproved packages on /distro/distroseries/+queue: I don't quite recall what they're like, as I've only seen them on one occasion, but they're different from the rest.

At least the terminology should be unified, but preferably the format too.
