Duplicate work items generate no error, only one shown

Bug Description
When editing work items, you can have two work items:
[person] Task: Status
Task: Status
The two tasks are identical. After closing the edit box, only one remains. It may that the last work item of a particular name wins and overwrites others (a bit of experimentation seems to indicate this). No error is generated.
