EEschema BOM not created correctly

Bug Description

When creating a BOM within EEschema usin plugin bom_csv.xls the quantities show up as 0.
I am using the windows release 4.0.0 I have tried this on a design of my own and on one of the demos. Found the same issue.
On the forum at least one other user has had the same issue.

I can confirm this on the lasest development build,

xsltproc --version
Using libxml 20903, libxslt 10128 and libexslt 817

bom_csv.xsl no longer exists. The replacement, bom2csv.xsl doesn't generate quantity columns at all.
