WSGI.middleware do not catch exceptions

Bug Description

WSGI.middleware do not catch and report exceptions like done for Application. and as a side effect this make the exceptions.* unavailable for middleware.

    catch errors in wsgi.Middleware.

    - Catch errors in wsgi.Middleware like done in wsgi.Applications.
    - Add tests for test_middleware.
    - Fixes bug 1128379.
