When clicking inside an inactive window it does not raise although it is configured to do so.

Bug Description
Binary package hint: kwin
I have set Window Behavior, focus, policy "Click to focus" and Window actions, Inactive Inner Window, Left Button "Activate, Raise & pass Click". At first it works fine. Then I make some changes in the configuration and when I change Desktop -> Desktop Effect -> Advanced -> XRender Options and activate "Smooth scaling", then clicking inside any window (Except Firefox!) passes the click but does not activate or raise. Deactivating "Smooth scaling" does not solve the problem.

Can't reproduce this here on KDE 4.2.1. What version of KDE are you using? What version of Kubuntu are you using? What graphics chip do you use?
Also, please, next time you report a bug, please format the description, I had a jolly hard time finding the settings (besides them being default anyway). Something like the following would have been awesome :)
Right click window title bar => configure window behaviour =>
Focus:
* policy: click to focus
Actions/Window Actions:
* left button: activate, raise & pass click
Desktop Effects/Advanced:
* XRender Options/[x] smooth scaling
It also worked out the way you wrote it, but it took me more time, so if you want to keep a poor developer happy, please use some formating like above :)
