All of the formish templates were passing the page_title directly into the page template, whereas the rest of the application expects the page_title to be set on the template API object. Changed the formish views, templates, and associated tests to use the API object like the rest of KARL (r4095).

Just a note that the original commit was accidentally performed on a KARL tag. I reverted the tag in r4907 and re-committed the changes to the trunk in r4908.
