Bug Description

I launch a service template using Configure->services->service templates. I add a service instance using that service template. The launch fails because there are multiple images of the same name on the node. No /var/log messages are logged for this failure.

root@nodea5:/var/log/nova# nova image-list | grep ubuntu-in

| 6709b57b-0115-42ac-b6b6-164dcb6d7634 | ubuntu-in-net | ACTIVE | |

1. I launch an template with ubuntu-in-net, and there are 2 copies of the image.
2. I add a service instance, and after staying at "spawning", the SI goes to "Inactive" after some time.(attached the snapshot)
3. No log messages in /var/log/nova, /var/log/neutron and /var/log/contrail.
