Manual provider does not clean-up HA controllers

Bug Description
HA in the manual provider is not yet possible without intervention (bug 1642618). When it is corrected, the clean-up script that tries to return the manual controller machine to a pre-Juju state only runs on the bootstrap instance, leaving the other HA controllers running with Juju agents and MongoDB instances.
