As of 2017-01-10 we see an ambiguous error
   failed to bootstrap model: refreshing addresses: An internal error has occurred (InternalError)
Other cases like this clearly indicate issues with google, azure, or openstack. I assume this "InternalError" is aws because all the failures were in aws at about the same time in us-east-1 and in a few other regions.
