Make it possible to re activate an existing user

Bug Description

After a user has been deactivated it should be possible to add a new user who has the same name.

The underlying user should actually be different. Deactivating a user should still be a one-way function

In particular, I think we should probably not be specifically "reactivating" a user; but we should be storing users by ID, and allow a new user to be created with the same name as a deactivated one.

It is considered to be more secure to create a user with different name once a user name has been de-activated. Non-repudiation is very important to enterprise as well as other companies that undergo auditing and scrutiny.
In fact, in some companies, even when the same employees are returned/re-hired, they would be given a new user name.
