It is fixed in 0.47 (Bug #168800 - export clipped paths to ps/eps or pdf fails).
You can't. It only works with vector objects. The only way to crop a bitmap in Inkscape is the clip method. But you can also modify your image with another tool (gimp?) before importing it in Inkscape.
