Not reproduced with r12991 and latest r13026 on OS X 10.7.5 with default (new) prefs, locale en_US.UTF-8, using the attached sample file and when recreated from scratch in new document.
Erratic behavior reproduced after changing default preferences to always use 'preserved' transforms (r12991, r13026). Can you confirm that the behavior is related to how transforms are stored?
