Inkscape Version: 0.48.3.1 r9886
The union operation fails in conjunction with stroke-to-path and skewing.
How to reproduce the bug:
Draw a thick line with "round" cap style. Apply "stroke to path" (CTRL+ALT+C) on line. Apply some skewing. Unite the result with a copy of itself (STRG++). After that, the path of the former caps looks really broken.

Reproduced with 0.48.2 and 0.48.4 on OS X 10.7.5
Not reproduced with latest trunk (r12763).
@Alvin - is this related to the elliptical arc issues you have been working on lately?
