Bug Description
Employee Record (with all the data fields of the paper form) report was split into 6 parts for easy export and printing. These reports can be seen in the reports section of configure system as follows:
Reports By Category (Kindly arrange it in order).

Employee Record Part 2 -- Part 2 of Complete record of each employee of Government of Bihar

Employee Record Part 3 -- Part 3 of Complete record of each employee of Government of Bihar

Following issues needs to be fixed.
Add Permanent Address (home state/district/
Remove Identification Type and number data columns from part 1 of the employee record
Add Mother's first name, middle name and surname in part 2 of employee record
Add Identification Type and number data columns in part 4 (currently) of the employee record after work contact details
Rename Employee Record Part 3 as Employee Record Part 4
Rename Employee Record Part 4 as Employee Record Part 3
Part 5 of employee record has position information module. This has more than 12 position details (for some employees) but need to show all of them in the report. Reason for Change data column (for each position) is empty and should be next to the position column.
All report table must show data if entered.
