Incorrect exception for open-output-file

Bug Description

&who should be open-output-file, and the exception should have condition type &i/o-file-already-exists, and since &i/o-file-already-exists is an &error, I think the &error is redundant. I'm guessing these issues might apply to other file operations.

This bug report is about to be closed as the fix comitted
previously will be incorporated in the next 0.0.3 release of
Ikarus Scheme, scheduled for January 31, 2008. A release
