Bug Description
The Disciplinary Action Module doesn’t make sense. There is a field called 'Action Taken' and the choices are: Assessed Mentally Incompetent, Death of a Patient… This field should be called 'Reason for Disciplinary Action' and there should be another field called 'Action Taken' with options like: Probation, Warning, etc. This should be changed for the demo.
