Bug Description
Disciplinary Action is not tied to any particular training and should be moved to the main menu of the individual as a separate form and the action taken (if any) be visible on the person view

Currently disciplinary actions are linked to trainings. Do you just want to see any current disciplinary actions on the person's page? Or do you want to actually assign the person a disciplinary action not linked to training?
