this is to capture present employment - the name and address should be text fields that would rarely go into any report but the location (state and lga) would be part of reports of geographical distribution of health workers and should be drop down lists to facilitate this

When setting the employment information, location and name of the health facility are as indicated. There is no address field. Should one be added?

Does a current employment report need to be generated? Or should the license or registration reports be modified.

Please clarify.

Usually, the report to be generated from this information would not need the exact address of the place of employment but the geographical location so as to be able to have an over of the geographical spread of currently licensed health workers in the country. Hence an address field would be useful but more useful is the location of the facility (LGA or State) as a drop down (mapped) field.

This is what I see now for the deployment information. It is a already a drop down (tree selection) of the location.

Did you mean to add a drop down in a different place?

Carl,

This is fine as in the attached image except to change the
"Deployment" on the Form title and the Deployment Date field label to
Employment.
