Long Image name breaks 'Create An Image' window layout

Steps to reproduce:

Login to Horizon as an admin
Navigate to Project -> Compute -> Images
Set Name -> win
Set Image Source -> Image File
Select Image File with name:
"9600.17050.WINBLUE_REFRESH.140317-1640_X64FRE_SERVER_EVAL_EN-US-IR3_SSS_X64FREE_EN-US_DV9.ISO"

Actual result:
Long Image name breaks 'Create An Image' window layout (see attachment)

Expected result:
Long Image name doesn't break 'Create An Image' window layout

Couldn't reproduce on chrome with master horizon. What version of horizon and browser do you use?

Horizon master-branch - 'Already up-to-date'. I don't see problem for Chrome. Problem is for FF 39.0

    Add 'width' for name-field

    In FF long Image name breaks 'Create An Image' window layout.
    In Chrome all looks good. Added 'width' for name-field,
    for fix problem in FF.
