Launch Instance > boot from image (creates a new volume) broken

Once you select Instance Boot Source as "Boot from Image (creates a new volume)" you expect another dynamic dropdown menu, but nothing happens (see [A] in image)

When I press Launch, it stays on the modal, but appends the other tab content to the first tab ("Details") (see [B] in the image)

At first I thought it was because I exceeded my volume quota, but that doesn't seem to be the case.

    Fixing the instance boot source

    An extra comma in the update_launch_source_displayed_fields
    brakes the launch instance workflow when the option selected
    is "Boot from image (creates new volume)"

Looking at the fix, the same code exists in Icehouse: https://github.com/openstack/horizon/blob/stable/icehouse/horizon/static/horizon/js/horizon.instances.js

This should be backported. (It seems to be in Havana as well.)
