LBaaS. HealthMonitor representation should be improved

When associating HealthMonitor to a Pool, theu user needs to select the HealthMonitor from a list.
In this list the HealthMonitor is repesented by its UUID. This is not very informative ..

We need to improve the HealthMonitor representaion and use some of the HealthMonitor attributes instead of the UUID.
