Create policy target based on instance name

When creating the PT here:
https://github.com/stackforge/group-based-policy-ui/blob/stable/juno/gbpui/panels/policytargets/workflows.py#L372-L373

it will be helpful to assign a name based on the VM. This will help in debugging to correlate the PT with the VM (especially in cases when the VM creation fails and we are trying to find out which PT was created for this VM).
