Bug Description
When a medication is discontinued and later prescribed again it is useful to have the reason why the medication was discontinued.
When a patient enters and leaves the hospital they often come back with a different medication because hospitals do not carry all meds. When viewing changes over time it might be helpful to know if the change was due to a hospital stay or medical condition.
When re-prescribing a previously prescribed drug the prescriber should be presented with the note so she can proceed or abort the task at hand.
