Install and configure (Ubuntu) in glance

I'm reporting a documentation bug.

Chapter prequisites has 2 §1.
the first §1. concerning mysql is uncomplete. It just says to connect to mysql but not what todo.

I suggest copy/paste instructions from previous version concerning
mysql CREATE DATABASE galnce;
mysql> GRANT ALL PRIVILEGES ON glance.* TO 'glance'@'localhost' IDENTIFIED BY 'GLANCE_DBPASS';
mysql> GRANT ALL PRIVILEGES ON glance.* TO 'glance'@'%' IDENTIFIED BY 'GLANCE_DBPASS';
