Bug Description

I tried to install fresh Fuel9.0 and after that update it to 9.1 version.
Platform Windows 7.0 + VirtualBox 5.1.6 r110634 (Qt5.5.1)
I used for this installation our vbox scripts from here [1], with one edit: I used 32GB config with 3 GB per node.
After successfully setup networks, bridges, nat and etc, fuel master node was started.
Anaconda was loaded without any problems and packages were downloaded [2]

But after some packages were installed, VM was rebooted and went to the maintenance mode (see screen attached). And installation was aborted by timeout.

If you interested in that lab when I tried it, please, contact me I will send all needed info.

1. To install 9.0 version of MOS you should use exactly 9.0 version of the scripts and the VirtualBox.

2. Please refer the official documentation: https://docs.mirantis.com/openstack/fuel/fuel-9.0/quickstart-guide.html#supported-host-operating-systems-and-virtualbox-versions
and don't use the latest version of the VirtualBox binaries. 5.0 version can be found here: https://www.virtualbox.org/wiki/Download_Old_Builds_5_0

3. Please use scripts without modifications (we don't have 32Gb option in the release), and do not configure VirtualBox manually before start scripts to test installation.

Anyway, we should support new versions of VBox.
I used the stable/mitaka branch from [1], without any modifications before starting launch_16GB.sh.
Master node deploy failed with error from the original bug description.

    Disable audio totally

Since VirtualBox 5.1 released on 2016-07-12 the audio setting enabled
by default which can be an issue on some hardware because audio not
    supported in the Fuel.
