It looks like the issue with deployment.

astute.log says:

Cluster[]: All nodes are finished. Failed tasks: Task[ironic-conductor/1] Stopping the deployment process!

ironic-conductor log contains:

ERROR oslo_service.service OperationalError: (_mysql_exceptions.OperationalError) (1045, "Access denied for user 'ironic'@'10.109.10.5' (using password: YES)")

    Fix ironic-conductor task dependencies

    The ironic-conductor requires access to the ironic database which is
    done in the ironic-db task. Previously we were just waiting for the
    database task but we should require the ironic-db task which waits
    for the database tasks to be completed and sets up the ironic database
    user permissions.
