The last item in the playlist, if removed manually, will be played again regardless of new playlist item

An item is added to the playlist (Track A).
Track A is commences playing (user presses play).
Track A is removed (or is stopped and then removed; or 'Clear All' is pressed), leaving the playlist empty.
A different item is then added to the playlist (Track B).
When play is pressed and Track B is expected, Track A will play instead.

Conditions:
If an item reaches its end naturally and is removed automatically, this situation does not occur.
This only occurs for the last item in the playlist. If an item is removed from a playlist, leaving remaining items, behaviour is normal.

Resolution (to return to normal behaviour):
Add the offending item into the playlist (Track A).
Add another different item (!Track A) to the playlist (if one is not already present).
Remove Track A from the playlist.
