Bug Description

When a patron cancels his/her own hold, create events for the related A/T hook "hold_request.cancel.patron". The hook is already present in the database. It just needs some action hanging off of it.

Tested and pushed to master and rel_2_1. On the fence about whether to bother with rel_2_0.
