modal_xulG_stack unreliable and new Operator Change features

The branch below has two commits.

The first rips out modal_xulG_stack and replaces it with openDialog calls instead, because I noticed that modal_xulG_stack wasn't working reliably. This could be considered a bugfix and may be backportable as a result.

The second adds a new dropdown to the operator change dialog, allowing the selection of a Temporary, Staff, or "Permanent" authtoken. The first two correspond to "temp" and "staff" authtoken durations, the latter is also a "staff" but disregards the previous authtoken instead of keeping it around. This is most likely 100% new feature territory despite some of the other minor fixes hiding within it.

Tom Berezansky suggested an issue we are having may relate to this.

Library Settings we have:

Idle Timeout: 22000
OPAC Inactivity Timeout: 22000
Staff Inactivity Timeout: 3600
Patron Login Timeout: 300

Some library staff report that that the OPAC times out on them in less than 1 hour when inside the client but they are not timed out of the client. Therefore, staff can go and look up a patron record, check an item status but are logged out of the TPAC and have no options to for actions for this record.

Tested various forms of operator change, authtoken time outs, and various dialogs. Looks good! Merged to master.
