missing prefix field in patron registration screen

Evergreen version: 2.0+

Apparently somewhere along the way, the "new" patron registration screen that came in 2.0+ does not include access to the prefix field of the actor.usr table. This was traditionally where values like "Mr." and "Ms." were stored.

The current patron editor only has a field called "Suffix / Title" now, where folks on our system have been entering both values, such as "Jr.", "Sr.", along with "Mr." etc.

Would be best to add back the Prefix option and then adjust the label for Suffix to be less confusing?

I believe I've got a branch that should repair this. Thanks to tsbere for the help with the show/require/suggest portions.
