TPAC users demand the ability to reset their passwords

There are currently some commented-out references in Open-ILS/src/templates/opac/parts/login/form.tt2 with a TODO about password resets, etc. Users will need the ability to reset their forgotten passwords, and there is an existing, usable password reset request form, so let's just add a simple link to that for now.

Pushed user/dbs/lp852341_tpac_password_reset in the working repo to implement this basic level of password reset functionality.

I'm looking at what it will take to merge password-reset into the TT-land (scrapping dojo, using locale::maketext, etc.). Will push new branch here when complete.

Bill, the integrated form looks great. Nice work!

I noticed that the password reset email template had a space on the blank line after then subject, which screwed up the resulting email (first paragraph tacked onto the Subject), so I added a fix for that into the same commit in which I created the upgrade script.
