Just to add to it. I was able to trigger this by logging into a brand new staff client session, recalling an item in the catalog, going to Actions for this record > Add Volumes and was able to trigger this error. I can't seem to trigger it at will though and I'm not really noticing a pattern right now that will force it to happen.
