Bug Description
Spinning this off from bug 1916403.
These two serials admin interfaces, both of which are still in AngularJS as of 3.13, have accessibility issues:
Serials Templates - org unit selector, as well as the create template modal
/eg/staff/
Serials Admin -> Prediction Pattern Templates - create record modal
/eg/staff/
