Improvements for the Circulation Policies modal

Follow up UI issues for the angularization of the Circulation Policies interface in 3.13 (bug 1855781):

1) The Unset option for Early Renewal Extends Due date appears, but a value of Yes/No is required or else it will fail to save without indicating why.

2) Linked Limit Sets would be less confusing at the bottom as it was in the old version.

3) I would like to see Description moved higher in the list so that it's easier to see which Circ Policy you are currently editing.

4) There is a gray box outline around the Circ Limit Set Name combo box that doesn't need to be there, and the Add button isn't aligned with the combo box.
