Creating a hold through Item Status > Actions > Request Items fails when using Honor user preferences

Bug Description
Tested in 3.9 and 3.10,
When viewing the item status, going to actions and selecting Request Items, when using "Honor User Preferences" it will fail to tp correctly pull the sms_carrier from the user_settings and instead just use 0, which causes the database to fail and the hold not be created.

I have experienced this failure in 3.7 also.
To the user, it's not clear why the hold is failing, the error is "The attempt to write to the db failed". Unchecking "Honor User Preferences" allows the hold to be created.
