Staff catalog search form uses placeholders instead of labels

Bug Description

In the staff catalog search form, most form fields lack labels. This requires users to remember what the field was for once the placeholders disappear after they've started filling in information. It also causes problems for screen reader users. Bug #1831489 describes the same issue in more detail for patron-facing screens.
