Bootstrap

If you use date of birth as a display field in your self registration form and set an example (library setting - Example dob field on patron registration) the example displays beside and below the date of birth field. Ideally the example should only display once.

It's not only the DOB example. Any other field with example text, phone, email, and postal code also display twice. A proposed fix is at https://git.evergreen-ils.org/?p=working/Evergreen.git;a=commit;h=1d5e98755f1f823a378f4cb7472ab0e1815fb1bf

I, Gina Monti, sign off on this bug. If DOB is required and a custom example is written as a value, the example will show and nothing else. Additionally, if a custom isn't included, the format will show besides the form field.
