3.8, 3.7 (probably earlier)
It is possible - though I imagine probably not common - to edit a system-generated penalty either via the Messages interface (3.7 and earlier) or the consolidated patron notes, alerts, blocks, and messages interface (3.8 and later via bug 1846354).
Editing a system-generated penalty adds a note component, and the note component doesn't auto-archive when the penalty is resolved. This leaves the penalty & note displayed in the upper part of the interface with active penalties, as well as the associated staff alert, even though the penalty condition is no longer actually active.
Additionally, in 3.8+ due to the new shared interface, it is now possible to edit the patron-visibility of system-generated penalties, which AFAIK have not ever been patron visible.
Due to the two cases above, I'm proposing that system-generated penalties be blocked from editing in the interface altogether.

I can confirm this is still happening in 3.11.2. Library staff edited a Patron Exceeds Overdue Threshold and it didn't clear when the situation was resolved.
