Screenreader Form Fix - Serials Templates (AngularJS)

There are several screenreader issues with the Serials Template form - headers aren't dictated, the buttons show up too early, the template name input is read simply as “edit,” and the information that is dictated is not entirely relevant or necessary(“Combo-box collapse” with no information about the combobox, and “Radio button 3 of 12,” for example).

This bug is specifically for the existing AngularJS UI for Serials Templates. Any Angular Port of the UI will need to address this issue.
