Self registration - problematic display of field on mobile devices

Bug Description

On a mobile phone when portrait oriented, the label and the field of the self registration form are placed inline. This arrangement causes the fields to be too narrow. The User cannot see what (s)he has filled in and the filled in values cannot be reasonably displayed and checked before submitting the form (see the attached screenshot).

It would probably be better for a device with a small display width to place a label above the field. This would make enough room for a sufficient field width.

The interface was tested in Bootstrap OPAC, Evergreen 3.7 beta.
