Request Item Functionality Doesn't Work Well When Some Default Preferences are Not Set

LP Bug #1841270 added the ability to honor hold preferences and place title-level holds from the Request Item action in Item Status.

The "Honor user preferences" could be more intelligent. Ideally the checkbox should behave the same as the catalog.

If the option is checked, and the patrons has no default pickup library, the pickup lib box defaults to blank in my testing. Instead it should rely on the Library Setting "Workstation OU fallback for staff-placed holds" to determine the pickup lib.

There are plenty of cases in our system where a patron may have default notification settings checked but no default pickup lib. Staff would have a difficult time using the feature as it works now.
