OPAC: Place hold fails silently if no pickup location selected

In the 3.6.1 Bootstrap OPAC:

If you try to place a hold without having a pickup location selected, the hold does not go through and remains on the page, but there is no error message or visual indicator why the hold was not successfully submitted.

This doesns't happen frequently, since most patrons will have a default pickup location, but it can occur if the patron's preferred pickup location is not allowing pickups. For testing, try to place a hold as the admin account which does not have a default hold pickup location set.
