Boostrap OPAC: Lost ability to have notes in Lists

In 3.6.1 Bootstrap OPAC:

To duplicate problem:

1. Create a list in My Lists
2. Add some items to the list
3. View the list and click the Edit button in the Notes column header
4. Enter something into one of the notes fields
5. Click the Save Notes button
6. It gives a "Problem with list management:" error on screen.

The browser console says "Failed to load resource: the server responded with a status of 404 (Not Found)"
