Rethink OPAC hold placement page layout

Suggested changes based on feedback from a usability study done by Mercer University:

The user groups found the hold placement form confusing - there are a lot of fields and they didn't necessarily understand all of them right away. In addition, none of the user groups even noticed the Advanced Hold Options. We should consider alternative options for the page layout - perhaps break the fields up into visual / color-coded blocks and add on-screen guidance?
