Survey options that don't appear to do anything

The most recent documentation I could find for surveys indicated that the "OPAC Survey" and the "Poll Style" checkboxes aren't implemented.

It doesn't appear in my testing that the angularization of the survey interface in 3.5 adds functionality for those pieces either.

I'm not sure what the Poll Style checkbox was originally intended to do, but adding the functionality for the OPAC survey sounds really useful.

I agree that this could be a really useful tool for libraries interacting with people accessing their OPACs.

Changing this from Wishlist to a bug, because options shouldn't display when they aren't tied to any functionality.

Ideally, the functionality would be added, but in the meantime, the options shouldn't display to the user.
