Configurable patron auth improvements

In bug 1817645, a couple of minor issues were identified for configurable patron authentication and retrieval:

- the 'allow inactive' option doesn't work. Looks like non-SUCCESS returns from open-ils.auth.login may need to be checked more precisely.
- a SERIAL primary key on config.remoteauth_profile would be good rather than the natural one so that eg-edit-fm-record can handle changing the name of a rule.

This bug is for tracking those requested fixes and any other small changes required to get from beta to official release.

Force-pushed a small syntax fix for the SQL upgrade script. Otherwise works fine for me. Removing myself as assignee and adding a pullrequest.

With the addition of EZProxy and PatronAPI support in 3.6 beta, the working branch needed a slight update. Here's a new branch, rebased to master and ready for 3.6:
