Bug Description

The fields when creating a new subscription are all over the place, they should be horizontally aligned

It seems like this only occurs at certain browser widths, but it could be improved

Confirmed in version 3.7. I agree that the display of the fields could be improved, and I needed to reduce my browser width rather significantly for the horizontal alignment.
