Add "Activated By" info to purchase orders

I'd be interested in having "Activated By" info on purchase orders, similar to Bill's work with Closed By/Close Date on invoices. It should also be reportable. I would imagine it would be somewhere at the top of activated purchase orders, something like "Activated by [user's username] on [date] [time]".

If there are multiple Acq users in a library, having a record of who activated an order can be useful. Additionally, many of our libraries require a supervisor's/director's approval to approve POs before ordering since, especially with EDI ordering, activating the purchase order puts the library under an obligation to pay for those items. If the supervisor/director was so inclined, they could activate the PO themselves and have it logged that they were the person to encumber those funds.

This also just helps to close the loop of accountability, since now there is info on who receives items and closes invoices.
